It happens. Once, I recorded the underside of a piano lid instead of the harp. Luckily all the reflections coming off the lid made a decent recording.

I recorded a singer once with a cardioid pattern on a Neumann U89 and it was turned around backwards. Even off-axis it didn't sound that bad. The singer sucked anyway so I'm not sure it would have made a great deal of difference facing the right direction. I noticed that it sounded funny but, left it that way. I realized it when I was putting the mic away. Nobody else noticed so I never said anything.

Yesterday I was in a hurry to record a guitar part myself. I was monitoring with phones and pushing the gain up thinking "why is this signal so weak on this loud guitar cab"

Well the mic was still over in the corner of the room no where near the amp.

^Hah, I've had a couple "moments" recently where the source of my confoundment was that I simply forgot to plug in the guitar. Every now and then I'll open up an old project and find errors, like a track not routed into the intended buss. But forgetting to plug in a guitar, well maybe someone should just take away these toys, lol.

That's actually a fair mistake if we are talking Pro Tools. Their bussing is the most powerful, but also the most complicated. I've done that a lot.

...and I've also screwed up the whole alt/option/command/shift clicking commands in PT when it comes to bussing tracks. Instead of bussing all the selected to a single buss, I've inadvertently sent every single track to its own buss ouput in sequential order. Talk about having to undo. Some of those actions aren't available with command+z either.
